Which insurance plans do you accept?

At Sports Medicine Consulting, our goal is to make healthcare accessible to you. We accept most major medical insurance plans including Aetna, ANTHEM/BCBS, Cigna, Humana, Medicare, United Healthcare, and Tricare. We are also pleased to announce our participation with CareCredit!

If you do not see your insurance plan listed, please contact our office to verify our participation.

Is a referral required for scheduling?

This is dependent on your individual Insurance plan. Please check with your insurance company if a referral is required for you to see a specialist.

What should I bring to my appointment?

Please bring your:

1. Driver’s License or Valid ID

2. Insurance Card

3. Reports, X-rays, MRIs, CT scans, etc. and any other relevant information.

You will also receive paperwork via email at the time of scheduling. The forms must be completed prior to your appointment.

How can I make an Appointment?

Please call us 770-713-6480 to book an appointment directly with our staff. You may also request an appointment online and one of our office members will contact you to schedule. Please note that your request is not confirmed until speaking with an office member. Once your request has been submitted, please allow 24 - 48 hours for your request to be processed and confirmed.
